FSSAI License (State) / FSSAI State License : This license is applicable for mid-sized companies with annual business turnover between Rs. 12 Lakhs to 20 Crores.
FSSAI License Annual Registration Fee is Rs. 2000 / Rs. 3000 / Rs. 5000 based on the kind of business and other factors.

Important Note for FSSAI License Renewal:

FBO's who want to apply for fssai license renewal must apply for renewal at least 30 days prior to the license expiry date to avoid late renewal charges i.e Rs. 100 per day.
Please note after license expiry date, Rs. 10k Late Renewal Fee Penalty Charges might be applied if you want to renew license in addition to Standard License Registration Fee amount.

Blueprint/layout plan of the processing unit showing the dimensions in metres / square metres and operation-wise area allocation
List of Directors / Partners / Proprietor / Executive Members of Society / Trust with full address and contact details with nomination of authorized signatory
Name and List of Equipments and Machinery along with the number, installed capacity and horsepower used
Analysis report(Chemical & Bacteriological)of water to be used as ingredient in food from a recognized/ public health laboratory to confirm the portability indicating the name of authorized representative of Lab who collected the sample and date of collecting sample. (Not Applicable if water is not being used as ingredient)
Photo I.D and address proof issued by Government authority of Proprietor / Partner / Director(s) / Authorised Signatory.
Proof of possession of premises. (Sale deed/ (Own Property) / Rent Agreement ) with latest Electricity bill, / Water Bill etc.,
Partnership Deed / Self Declaration for Proprietorship / Memorandum & Articles of Association (AOA) towards the constitution of the firm / Copy of certificate obtained under Coop Act-1861/ Multi State Coop Act-2002 in case of Cooperative
